Children’s Minnesota advances community health

Children’s Minnesota has a mission to champion the health needs of all children and families in our community. To pursue that mission, we are creating sustainable change within our health care system and actively partnering with the communities we serve to foster trust, develop inclusive programming and help bridge the gap in health outcomes we see every day. This includes:

  • Through our Collective for Community Health, we partner with the community on innovative solutions that eliminate health disparities and advance health equity . Work includes unique programs such as:
    • Community Connect and the Healthcare Legal Partnership, which help connect patient families with social and legal services to address health-related social needs.
    • Collaborating with the health equity team to enhance access to care through a mobile clinic, providing life-saving childhood vaccines to children in schools and local community social needs (like housing, food and legal support).
  • Advocating for public policies and systems changes that improve the health of children and families.
  • Transforming the foundation of our organization with equity and inclusion best practices to better meet the needs of our patients and families and create a more inclusive culture to recruit and retain a diverse workforce.

This work is an integrated part of the Children’s Minnesota strategic plan. Community health strategies are guided by:

  • Our Community Health Needs Assessment, which brings together community representatives across sectors with Children’s Minnesota leaders to guide our long-term work in improving the health of the children and families we serve.
  • The Children’s Minnesota Community Advisory Council to ensure representation of diverse community voices.
  • Health equity metrics that account for the interrelatedness of clinical outcomes within our organization and social determinants of health, guiding our improvement initiatives both internally and externally with our community partners.
