Children’s Minnesota uses technology to address social determinants of health

Children’s Minnesota uses a platform called NowPow to address social determinants of health by matching community resources with patients in need. Jessica Block, manager of community health programs at Children’s, explained to Children’s Hospitals Today that leveraging this database allows Children’s to consistently refer patients to resources based on the patient’s address, gender or preferred language. Furthermore, using this technology allows Children’s to hire staffers based on “soft skills” rather than familiarity with local resources.
