Notice to parents of children who are 18 years of age or older:
Children’s Hugo Clinic will need written consent from your child in order to communicate with you about all of your child’s medical needs. Please see the front desk staff to complete this form.
If parents will be out of town, Hugo Clinic will need written consent from the parents stating who is taking care of their children while gone and that you give your permission for treatment of your child(ren) in your absence. Please see the front desk staff to update your Annual Consent Form.
